CHECKLIST: Ergonomic Evaluation Process

Summary: Be on top of your Ergonomic Evaluation Process (flowchart) by following the steps below.

Steps Procedure
1: Initiate Department Notification
  • Individual employees complete and submit Department Notification Form on-line
  • Supervisor and other Department contact will be notified upon completion of the form
2: Self-assessment & Microlearning
  • Employees complete and submit self-assessment for EHS review
3: Results from Self-Assessment
  • EHS reviews self-assessment responses from employee
  • Employees and Supervisors receive self-assessment action list
4: Prioritize Posture
  • Employee's priority is to maintain and instill good posture throughout the workday
  • Employees incorporate stretches from Step 3: Stretch It Out! section of the action list
5: Adjust Equipment
  • Employees and Supervisors test equipment adjustability with instructions from the action list
  • Employees immediately apply some temporary solutions if equipment and work space lacks adjustability or lacks surplus equipment
  • Supervisors note areas of equipment improvements for budget and implementation plan
6: Implement Products
  • Supervisor and Department communicate implementation or budgetary concerns hindering implementation to employees
  • When purchasing, supervisors and department utilize and select from Pre-Approved Ergonomic Furniture and Product List to pursue Ergonomic equipment procurement
7: Status Update
  • Employees receive and Supervisors copied a Ergonomic Self-Assessment Action List Status on-line form via email
  • Employees complete and submit Ergonomic Self-Assessment Action List form
8: Further Assistance
  • Employees request an further assistance thru on-site or virtual ergonomic evaluation via Ergo Self-Assessment Action List form
  • EHS schedules on-site Ergonomic evaluations with Employees and notification to Supervisors
  • Or EHS sends virtual Ergonomic evaluations within 5 business days photos or videos are submitted via Ergo Self-Assessment Action List form

Note: Ergonomic evaluation services are at no cost to the department. Consider including your departmental ergonomic needs in your annual budget. Procurement is not guarantee and will be decided by the department. If any recommended products do not fit the needs or budget of a department, please contact EHS for further assistance in seeking alternative equipment recommendations.
