Checklist: Contents of a Staff Employee General File

Summary: A staff employee's general file contains personnel documents that have a direct relation to the employee's job such as licenses/certificates, performance evaluations, employment application, and records of salary changes. The general file is part of an employee's personnel record.

Access to the general employee file is limited to those with an immediate business need to know. It is recommended that the general employee file be kept with the payroll file but separate from the confidential and medical files.

  • State Oath of Allegiance/Patent Policy/Patent Acknowledgement (Original to Payroll)
  • Personnel Form Acknowledgement
  • Signed job description
  • Signed application (verify no near relatives and no felony convictions; if so, contact your recruiter)
  • Resume
